Transcript Request

IMPORTANT: Alumni must have a transcript/records release signature on file at MHS.

Transcripts/Records may be requested in writing on the attached release form accompanied by a clear photo copy of a picture I.D. and mailed to:

or via email to Mrs. Drury at drury@mentorschools.org. Once Mrs. Drury receives your request and validates your signature, she will process your request. If you have questions regarding your transcript/records request, you may email Mrs. Drury or contact her directly at (440) 974-5327.

Note: If you are currently attending Mentor High School, please see your guidance counselor to have your transcript sent. DO NOT USE THIS FORMAT.
